Accident Investigation

Inspection and regular consultations between management and employees will be able to prevent many accidents. If the accident is still going on, it needs to be investigated thoroughly to determine the cause. Most accidents and incidents caused by several things, such as:
 equipment damaged or impaired
 work schedule or production schedule is too crowded
 How it works is not right
 The existence of barriers in the usual path traversed
 liquid spilled on the floor
 Environmental factors, such as noise and working hours are too long
 Failure to implement safe work practices
 Training and supervision are inadequate
Incident means something that is likely to cause injury or illness, including an accident resulting in injury or likely to cause injury. Employees who report an emergency should explain what’s his name, where they are located, the type of emergency, as well as the number of employees involved.

Effective reporting of an accident investigation will yield information that is important for management in an attempt to determine the actions needed to prevent the same circumstances, to choose the priority of prevention, and to assess the effectiveness of the control systems that have been implemented.

What needs to be investigated?
All accidents or circumstances which are likely to cause accidents should be investigated as soon as possible after they occur. Site investigation should be carried out immediately and made a list of the chronology of events leading up to the accident, including the statements of witnesses. Each time the accident or incident, accident investigation form must be filled. The form should be specially designed so that its contents:
 To provide an objective assessment of the impact of the accident in terms of loss of manpower and cost
 Describe the actual cause
 Facilitate the identification of problem areas or certain danger
 Suggest remedial measures
 Providing information for statistical data collection and as consideration in analyzing the necessity of the training program
 Provides data on health and safety at work
 Provide information to support claims for loss
 Providing information to be published in the bulletin of health and safety. Or to prevent the same accident.

Note the things with regard to the examination of the following:
Upon the occurrence of an accident or incident, the management is responsible to take immediate action to improve the situation, and prevent the same accident from happening again. In this case the representatives of the management should consult with representatives of occupational health and safety are concerned. After the occurrence of the fatal accidents or severe, good location and everything connected with the accident, should not be disturbed.

Fortunately, most of the fires are generally relatively small and can be overcome by using a fire extinguisher appropriate. Rescue the human spirit must be done first before the fire is extinguished.

Prohibited areas
Restricted area is an area “not allowed in” because of the occurrence of something, for example the process of operation does not run properly, or there is a faulty equipment that pose a danger to the safety and health of employees who may be entering the area.
